CLINICAL PEARLS:
- Full thickness macular holes are classified by size, specifically the minimum linear diameter: Small <250 microns, Intermediate 250-400 microns, Large >400 microns.
- Small macular holes can respond to topical therapy depending on their specific configuration, though they often require ongoing maintenance/monitoring.
- Large macular holes usually require additional surgical maneuvers beyond ILM peeling and gas (such as ILM flaps, hydrodissection, amniotic membrane grafts and retinal autografts).
